FOR PEOPLE WITH HIGH BLOOD PRESSURE

The Blood Pressure Self-Monitoring (BPSM) program is a four-month evidence-based health intervention designed to help persons with high blood pressure better manage their blood pressure by developing the habit of routine self-monitoring and tracking their blood pressure. Participants work with trained Healthy Heart Ambassadors for the duration of the program and are encouraged to:

  • Measure and record their blood pressure on their own ("self-monitor") at least two times per month
  • Attend at least two personalized Office Hours consultations with a Healthy Heart Ambassador per month
  • Attend monthly Nutrition Education Seminars

The Y's Blood Pressure Self-Monitoring gives participants the knowledge and guidance to monitor their blood pressure and track changes.

The Blood Pressure Self-Monitoring program aims to:

  • Reduction in blood pressure
  • Better blood pressure management
  • Increased awareness of triggers that elevate blood pressure
  • Enhanced knowledge to develop healthier eating habits

The Blood Pressure Self-Monitoring Program includes:

  • Meetings with Healthy Heart Ambassadors for the duration of the four-month program.
  • Training on how to Self-measure your own blood pressure
  • Two personalized consultations per month
  • Monthly nutrition education seminars.
  • Options for participants who need a home blood pressure monitor.

Virtual visits are scheduled (via phone call or Zoom) with participants. Each visit is approximately 10 minutes. Two visits per month are program requirements.

Qualifications to Participate

Must be at least 18 years old
Must have been diagnosed with high blood pressure
Not have experienced a recent cardiac event
Not have atrial fibrillation or other arrhythmias
